Smoker's Lips: A Treatment Protocol for Ex-Smokers with Dark Lips

Quitting smoking is one of the best things a patient can do for their health. A few months later, many of them notice something they never paid attention to before: their lips are still dark.

That's often when they come to a dermatologist. They have made a hard change and want to see it. For clinics, these are some of the most motivated lip patients you'll meet, and a large share of them are men, a group most aesthetic practices barely market to.

Here's how to assess and treat smoker's lips, and where Pink Booster fits.

Why smoking darkens the lips

  • Melanocyte stimulation: compounds in tobacco smoke, including nicotine, stimulate melanin production in the lips and oral mucosa. This is known as smoker's melanosis.

  • Heat and repeated contact: the lips take direct heat and contact with every cigarette, bidi or hookah session.

  • Chronic dryness and irritation: smokers often have dry, dull lips, and the ongoing irritation adds post-inflammatory hyperpigmentation on top.

  • Less blood flow: smoking reduces microcirculation, which leaves the lips looking dull and less pink.

The pigmentation often shows on the gums as well, especially the front gingiva.

Does it fade after quitting?

Often it does, partly. Smoker's melanosis can lighten gradually over months to years after quitting, but many patients are left with residual darkening, especially if they smoked heavily or for a long time, or have darker skin types.

This is the gap where treatment helps. The trigger has been removed, so the results of a treatment course are far more likely to last.

Screen first: this matters more in smokers

Before treating anything cosmetically, examine the whole mouth. Tobacco users, including those who chew tobacco, gutka or paan, have a higher risk of oral and lip cancers and premalignant lesions. Look for:

  • White or red patches on the lips or inside the mouth (leukoplakia, erythroplakia)

  • Non-healing ulcers, cracks or crusting on the lips

  • Thickened, rough or indurated areas, especially on the lower lip

  • Restricted mouth opening or fibrous bands, which suggest oral submucous fibrosis

  • A solitary, irregular or changing pigmented lesion

Anything suspicious should be biopsied or referred before any aesthetic treatment. Gum pigmentation is treated separately; refer patients who want it addressed to a dentist or periodontist.

The assessment

  • Smoking history: what form, how much, for how long, and when they quit (or whether they're still quitting)

  • Pigment pattern: diffuse darkening is typical; focal or irregular pigment needs a closer look

  • Barrier status: dryness, flaking, fissures or cheilitis

  • Other causes: lip-licking, sun exposure, lip products and medications can all add to the picture

  • Herpes labialis history: plan prophylaxis according to your usual lip-procedure practice

  • Baseline photos: standard lighting and angle, which matter even more for patients motivated by visible change

Where Pink Booster fits

Pink Booster by Promoitalia treats both problems smokers' lips typically have, dryness and pigmentation, in one treatment:

  • Hyaluronic acid (15 mg/ml) in a range of molecular weights restores hydration to a dry, dull vermilion

  • Vitamin B12 and liposoluble vitamin C support repair and add antioxidant protection

  • Biomimetic depigmenting peptides, tranexamic acid, arbutin and reduced glutathione work on melanin production from several angles

Best candidates: patients who have quit, or are actively quitting, with diffuse residual darkening and a clear oral examination.

Set expectations clearly: hydration and softness improve early, while colour improves gradually over a course of sessions as per the Promoitalia protocol. Patients who are still smoking should know that ongoing smoking will keep driving pigment and limit how long results last.

Supporting the patient beyond the lips

  • Reinforce the quit. Treatment works best as a reward for quitting, so point patients to cessation support or a tobacco quitline if they are still struggling

  • SPF lip balm every day, since sun keeps adding pigment

  • A bland, petrolatum-based balm at night

  • No lip-licking, picking or scrubbing

  • Maintenance sessions as advised, to keep results stable

The clinic opportunity

  • Reach men: smoker's lips is one of the easiest ways to bring male patients into aesthetic treatment without it feeling cosmetic

  • Frame it as a milestone: treatment that marks a quit date, for example at 3 or 6 months smoke-free, gives patients a reason to book

  • Partner with dentists: dentists see smoker's pigmentation every day and can refer lip cases to you while you refer gum cases to them

  • Use before-and-after photos: with consent, these are especially convincing for this patient group
